Bring ALL of the Following to Your Appointment:
- Your child’s birth certificate
- Copy of your child’s latest physical exam
- Your child's immunization record and TB risk assessment results
- Proof of New Britain residency (phone bill, lease, etc.)
- Proof of all family household income for the most recent 4 weeks
- Evaluations for any special needs (speech, hearing, Birth to 3, etc.)
- Social security numbers for everyone in the family if available
- Proof of legal guardianship, if applicable
- Your child’s medical insurance card
